I'm not an expert, but when I installed this, the bottom of the extender sat right down over the stem and ended up flush with the top of the crown race, so as far as holding the bearings together goes, no spacers are needed. It may depend on the length of your stem.
The installation guide indicates that spacers should be placed below the handlebar stem, but I don't really see the point if the stem is clamped on tightly enough.

when I placed it on my stem I needed about 21mm extra spacer rings, so I am guessing a few extra wouldn't go astray.

Depends on the length of your steerer tube. You probably will but as wiggle does not charge postage you can try it out and then ordeer the necessary spacers later. I needed 3x10mm to set it as I wanted it

It comes with a star nut and a longer bolt. I did not use the star nut that came with it as the bolt reaches the existing nut in the tube.(I could not see how to use the supplied nut). I expect you could cut it to be a bit shorter as I used spacer rings to make full use of the extra height.

Yes this will increase the height or you could try this option also. Ascertain the stem length you have and match with the appropriate adjustable stem. Please be aware however that this bring you back slight when raised which may not suit you:-
